a waiver and registration form. On the bottom of the form please cross off the credit
card information and write GROUPON and your GROUPON BAR CODE NUMBER on the
bottom of your Groupon voucher or your Daily Deal, Living Social Voucher number in
the slot. You will not be allowed in the class without this information. Fax that
information to 1-888-226-8365.
Visit the Registration page and make a Pole Addiction Account. Send an email with
your contact information and Daily deal voucher number to deal@poleaddiction.com
after you sign in! Classes fill up fast and they are first come first serve basis. You can
also just email your selected class, contact information and groupon bar code or
voucher number if you do not have the ability to scan and fax.
Also if you have a girls night out / private party / bachelorette party and would like to
use your daily deal value of $125 toward a private party or Private lessons email

you can get in with our re-take discount of $75 for the series. Please email after your
class if you are interested in re-taking any of your pole or aerial series classes.
